Gastric tonometry: the hemodynamic monitor of choice (Pro).
Controversy exists as to the best means to monitor the critically ill patient and the appropriate end points of therapy. Use of global hemodynamic or metabolic parameters may be normal in the patient who has not been completely or adequately resuscitated. Decreased perfusion to the gut is not well tolerated and may contribute to the development of the multiple organ dysfunction syndrome. Gastri...
متن کاملGastric tonometry after gastroschisis repair.
Gastric tonometry and intravesical pressure measurement were performed on eight babies born with gastroschisis for 72 hours after abdominal closure. Intravesical pressure was not high. The gastric mucosal pCO2 and gastric mucosal pH remained stable and closely matched arterial values. Tonometry may be a useful technique to monitor these babies.
متن کاملResuscitation of a polytraumatized patient with large volume crystalloid-colloid infusions--correlation between global and regional hemodynamics: case report.
Aggressive large volume resuscitation is obligatory to achieve necessary tissue oxygenation. An adequate venous preload normalizes global hemodynamics and avoids multiorgan failure (MOF) and death in patients with multiple injuries. Large volume resuscitation is associated with complications in minimally monitored patients. A properly guided resuscitation procedure will finally prevent MOF and ...
متن کاملIntestinal and gastric tonometry during experimental burn shock.
INTRODUCTION The occurrence of organ failure following thermal injury, despite restoration of hemodynamic parameters and urine output during resuscitation, has led to efforts to measure end-organ perfusion. The purpose of this 24-h study was to evaluate the utility of gastrointestinal (GI) tonometry during burn shock and resuscitation. METHODS Male swine (n=11, 23.3+/-0.9 kg) were anesthetize...
متن کاملTonometry of partial carbon dioxide tension in gastric mucosa: use of saline, buffer solutions, gastric juice or air
Tonometry of gastric mucosal partial carbon dioxide tension (PCO2) has been forwarded as a clinically applicable tool to monitor regional perfusion adequacy during circulatory failure. The technique is still not used widely, partly because of methodological problems. Current measurement principles are reviewed, with help of the report on PCO2 measurements in gastric juice and tonometer in this ...
